Michigan schools and licensed childcare centers are required to report Immunization Program (IP) compliance on students through the age of 18 to their local health department.
Schools and childcare settings are required to report positive communicable diseases to us. Please use these forms to report COVID-19, influenza-like illness, Noro-like illness, and Chickenpox:
