Community Health Worker

The Benzie-Leelanau District Health Department is looking for a Community Health Worker to join our Community Connections team. Do you enjoy helping people and have excellent communication skills? Are you highly motivated and resourceful?

As a trusted front-line public health worker, you will establish relationships with individuals of all ages and backgrounds and help them access the resources they need. You need to be able to function independently and as part of a team, and be comfortable using technology. Meeting with people in their homes, as well as community settings, is an important part of this job. We provide services for individuals in Benzie, Grand Traverse and Leelanau Counties.

Required Qualifications

• High school diploma or equivalent
• Strong community and interpersonal skills
• Ability to build trust with individuals from diverse backgrounds
• Able to navigate Microsoft Office applications and electronic medical records
• Community Health Worker certification, or willingness to obtain certification
• Current Driver's license with reliable, insured vehicle

Preferred Qualifications

• Knowledge of community resources and assistance programs
• Experience working in public health, social services, healthcare, or community outreach
• Comfortable sharing program information with community groups

This is a full-time position with a generous benefit package including health insurance, health savings account, dental and vision insurance, a generous retirement package, and paid vacation/sick time.  Salary range $22-$25/hour.

Work Location: Hybrid remote, serving clients primarily in Benzie and Grand Traverse Counties
