Frankfort-Elberta Area Schools - School Wellness Program (SWP)

Frankfort-Elberta Area Schools' SWP has a strong focus on both physical and mental health, and the well-being of students.

Goal:

With a school nurse and a mental health provider, SWP can provide nursing services, health education, and confidential individual and group counseling in a private location.

Advantages:
  • Students can experience their care in a comfortable, familiar, and confidential environment
  • Less loss of class time and fewer disruptions of regular routines
  • Removes barriers to parent transportation and job-related issues that might hinder access to services
  • Expedited treatment leading to earlier identification and intervention for students’ health needs
Program Details:

The SWP is operated by the Benzie-Leelanau District Health Department, in partnership with Frankfort-Elberta Area Schools, with major funding from the Michigan Department of Health and Human Services and Michigan Department of Education.

There is no personal out-of-pocket cost for health services provided at the SWP. If health insurance is available, mental health visits may be billed as this helps to support the program. The SWP team can help with finding insurance and other community resources as needed.

Services are offered without regard to sex, race, religion, or sexual orientation. 

Parent consent required for students to receive services, except as exempted by law.

Nursing Services:

The nurse is available during school hours and can:

  • Assess and provide first-aid for illnesses and injuries
  • Conduct screenings and provide referrals for health services
  • Develop and monitor health plans for students with health conditions
  • Help to create and support health education for students, parents, and staff
  • Collaborate with students' primary care providers and specialists as needed

Elizabeth Merrill-Tietz, RN

    Hello FEAS families, I am your school nurse Mrs. Merrill-Tietz, I have worked in healthcare and our community for many years, this is my 3rd year as a school nurse. I enjoy working with Pre-K-12th grade and families. I am available daily when school is in session. When I’m not at FEAS, I enjoy spending time outdoors and traveling with my husband and 2 children.

    Crystal Thomas, LMSW

    Crystal Thomas, LMSW, is a Behavioral Health Therapist with offices at Frankfort-Elberta Area Schools. She completed her Master of Social Work at Grand Valley State University.  She is a Licensed Clinical Social Worker who has been providing counseling services to families and children since 2010. The program assists students with mild to moderate behavioral health concerns. Counseling services may include individual and/or group counseling for: anxiety, depression, anger management, social/relationship skills building, grief & loss, behavior management and expanding healthy coping skills.

     

    Services are by appointment. If you would like to meet with the counselor, please call or email and we will set up a time to connect.
