Sanitation & Foodborne Illness Complaints 

If you have a concern about sanitation or a complaint about the safety of food served at a facility in Benzie or Leelanau County, please let us know. Your report will be forwarded to our food program for follow-up. For us to complete a thorough investigation, please provide us with the following information. 

  • Date and Approximate Time of the Incident 
  • Description of the Problem 
  • Name and Location of the Establishment 
  • Who Was Involved (for Example, Cook, Cashier, Wait Staff) 
  • Your Name and Contact Information. Phone number or email is required in case our staff has additional questions. 

How to Make a Report 

  • Foodborne Illness complaint - please complete this survey  
  • General Food Complaint – please complete this form and submit it to bldhd@bldhd.org or in person at one of our offices. 
  • Call the Benzie-Leelanau District Health Department.
